ABSTRACT

Mefenamic acid is a non steroidal anti-inflammatory drug, has anti-inflammatory, analgesic, anti rheumatic, and antipyretic activity. It has low solubility in the stomach milieu. It causes ulceration and bleeding in the upper gastrointestinal tract. The aim of this study is to know ulcerogenic effect of solid dispersion and physical mixture of mefenamic acid-HPMC in comparison with pure mefenamic acid. This study used 25 Wistar male rats, 2-3 months age and 150-200 grams in body weight. The 25 rats divided into five groups randomly: group I was normal control, group II was HPMC as dispersing agent control which the dosage 360 mg/Kg BW, group III was mefenamic acid as drug control which the dosage 360 mg/Kg BW, group IV was physical mixture of mefenamic acid-HPMC which the dosage 720 mg/Kg BW and group V was solid dispersion of mefenamic acid-HPMC which the dosage 720 mg/Kg BW. Each group was orally given with 1% PVP Observation was done by examining the macroscopic features (hyperemia, petechiae, ecchymoses, and erosion) and microscopic features of rats stomach. The result showed that solid dispersion mefenamic acid-HPMC significantly reduced ulcerogenic effect of mefenamic acid 80,68% (p<0,05), compared to physical mixture mefenamic acid-HPMC 54,91% (p<0,05). In conclusion, the solid dispersion and physical mixture of mefenamic acid-HPMC have less ulcerogenic effect in comparison with pure mefenamic acid.

ABSTRAK

Asam mefenamat merupakan obat anti-inflamasi non steroid, memiliki aktivitas anti- inflamasi, analgesik, antirematik, dan antipiretik. Asam mefenamat merupakan obat yang kelarutannya rendah. Asam mefenamat menyebabkan ulserasi dan perdarahan pada saluran cerna atas. Penelitian ini bertujuan untuk membandingkan efek ulserogenik dispersi padat asam mefenamat-HPMC dengan campuran antara asam mefenamat dan HPMC serta asam mefenamat saja. Penelitian menggunakan hewan uji tikus putih jantan galur Wistar sebanyak 25 ekor, umur 2-3 bulan dan berat 150-200 gram. Dari 25 ekor tikus, dibagi menjadi lima kelompok perlakuan: yaitu kelompok kontrol normal, kelompok kontrol pendispersi HPMC dengan dosis 360 mg/KgBB, kelompok kontrol obat asam mefenamat dengan dosis 360 mg/KgBB, kelompok campuran fisik asam mefenamat-HPMC dengan dosis setara, kelompok dispersi padat asam mefenamat- HPMC dengan setara. Masing-masing diberikan secara per oral dengan pensuspensi PVP 1%. Pengamatan ulserogenik dilakukan secara makroskopis serta histologi jaringan untuk melihat perubahan mikroskopik pada lambung. Hasil penelitian menunjukkan bahwa dispersi padat asam mefenamat-HPMC dosis setara 360 mg/kg dapat menurunkan efek ulserogenik asam mefenamat sebesar 80,68% dibandingkan dengan campuran fisik asam mefenamat-HPMC sebesar 54,91 %. Dapat disimpulkan bahwa dispersi padat asam mefenamat-HPMC mempunyai efek ulserogenik yang minimal dibandingkan dengan campuran antara asam mefenamat dan HPMC maupun zat tunggal asam mefenamat.
